You should mention any other valid sources of income(if any). If anybody contributes a handsome amount Monthly/yearly and the family depends on his/her income then you should mention it otherwise not.

You should mention the age of their NID card.Put a round number like 32,45,57 etc…. If that’s difficult or unable to do, then mention an approximate age. It will not create any problem. But you must be accurate about your father and mother's age according to their NID card.

There is no relation between Medical problems and ISSB. A green card doesn’t depend on Medical problems. After getting a green card your final medical will be conducted. Final medical board will decide whether you are physically fit to join the academy or not. It is necessary to write all the truth about your health condition because if you get a green card and they find a mismatch between your biodata form and your current medical report then it will create a vulnerable situation for you.
